For those requiring in-patient care following surgery, injury or illness, Creekview Rehabilitation Center on the campus of Evergreen offers 24-hour short-term skilled nursing care and therapy services. All services are designed to help patients return home as quickly as possible.
Short term rehab and therapy at Creekview Rehabilitation Center is available to residents of Evergreen retirement community on a priority basis, as well as to people living in Oshkosh and surrounding cities.

- Medicare-certified, licensed skilled nursing focused primarily on short-term rehabilitation
- Located in Households 5 and 6 of Creekview South—close to Evergreen Therapy Services, the Fitness and Aquatic Center, Creekview Café and Uniquely You Salon
- Priority access to other living options based on availability
Daily Fees
- Private room $435/day
- Intensive skilled nursing $30/day surcharge
Medicare or insurance may pay for a portion of the costs listed above.
Wisconsin Bed Tax Assessment of $170/month is not included in the above rates
