Skilled Nursing

Short-term and long-term care

Evergreen's 80-bed, Medicare-certified licensed skilled nursing facility offers both short-term rehabilitation care, long-term care and palliative care.

The Creekview Rehabilitation Center at Evergreen provides care for 22 residents in two households for those who are focused primarily on short-term rehabilitative therapies and who plan to return home after recovery.

Creekview long term skilled nursing provides care for 58 residents in six households with a nationally-recognized innovative approach to long-term skilled nursing. Each household contains living space in an appealing setting, much like home.

Both Creekview Rehabilitation Center and Creekview long-term skilled nursing serve residents who need ongoing 24-hour skilled care and are staffed by nurses and certified nursing assistants 24 hours per day. In addition, other staff members include social services specialists, certified occupational therapy assistants and unit clerks.

Our nursing staff is well-educated and specially-trained in treatment measures such as wound care, comfort care, pressure ulcer prevention, and more. Skilled counseling by social workers is also available to residents and families.

  • Long-term skilled nursing care

    Creekview North and South offer 24-hour long-term licensed skilled nursing and end-of-life services in a home-like, household setting for those needing the highest level of personal care. Residents and their loved ones make choices about the quality of life through regular care planning meetings with staff.

    Creekview is a Medicare-certified facility. Nurses have received special education and training in areas including comfort care, wound care, pressure ulcer prevention, hospice care and more.

    Convenient parking near the Creekview entrance on Eagle Street provides easy access to the Creekview households as well as Evergreen Therapy Services, the Fitness and Aquatic Center, Creekview Café, and Uniquely You salon.

    The Creekview Rehabilitation Center at Evergreen offers 24-hour short-term skilled nursing care and therapy services for those who plan to return home after recovery. Creekview is a Medicare-certified facility.

    Located on Households 5 and 6 of Creekview South, the area is close to Evergreen Therapy Services, the Fitness and Aquatic Center, Creekview Cafe, and Uniquely You Salon.

    garden place loungeGarden Place loungeEvergreen Garden Place and Garden Terrace are state-registered Residential Care Apartment Complexes (RCACs). The 20 studio and one-bedroom apartments are designed for those who need minimal scheduled daily assistance during the hours of 6:30 a.m. to noon and again from 4:30 p.m. to 8:30 p.m.

    Garden Place and Garden Terrace residents must be their own legal decision-makers with no guardian or activated power-of-attorney. They must also be able to move about the building without staff assistance.

    Lounges provide areas beyond the private apartments for relaxation and entertaining. Enjoy the fireplaces, televisions, computers, and game tables. Access to the Garden Grove patio and seasonal gardens encourages outdoor activities. Parking is conveniently located near the Garden Place entrance.

    Evergreen Garden Heights and Manor View are licensed Community Based Residential Facilities (CBRFs). The 40 private accommodations are designed for those who require regular ongoing assistance with activities of daily living including one-person transfers (but not skilled nursing care). 24-hour resident assistant staff and a nurse are available at all times. Residents may be their own decision-makers, or they may have designated legal decision-makers.

    Shared living spaces in both Garden Heights and Manor View feature full kitchens and dining areas as well as comfortable living rooms. A lounge area which connects Garden Heights and Manor View overlooks the picturesque Garden Grove outdoor patio and gardens, and also leads directly to an outdoor rooftop garden.

    sharehavenShareHaven Evergreen ShareHaven is a Community Based Residential Facility (CBRF) providing memory and dementia care for those experiencing symptoms of Alzheimer's disease and other dementia. ShareHaven residents must be able to walk without staff assistance and do not need skilled nursing care. They must have a designated and activated legal decision-maker.

    This one-story home in a residential neighborhood is directly across the street from the Evergreen Manor Building and provides a quiet environment with minimal disruptions. Two households each contain a living room, dining area, full kitchen, and 10 private bedrooms with private bathrooms. The specially-designed floor plan enables residents to easily find their way through both households.

    ShareHaven is a safe, monitored environment with outdoor access into a large and inviting fenced-in back yard with seating, raised flower beds and walkways.

    As part of the Master Site Plan project, Evergreen plans to add at least 40 new independent apartments attached to the main Evergreen building. If all goes as planned, construction will begin in late 2016 or early 2017.
