Short-term and long-term licensed skilled nursing

Evergreen's 108-bed, Medicare-certified, licensed skilled nursing facility offers both short-term post-surgical and rehab care and long-term care. 

The Post Surgical and Rehab Center at Manor View provides care for 28 residents whose needs are focused primarily on short-term rehabilitative therapies. 

Creekview provides care for 80 residents in eight households with a nationally-recognized innovative approach to long-term skilled nursing. Each household contains living space in an appealing setting, much like home.

Both The Post Surgical and Rehab Center at Manor View and Creekview are designed for residents who need ongoing 24-hour skilled nursing care and are staffed by nurses and certified nursing assistants 24 hours per day. In addition, other staff members include social services specialists, certified occupational therapy assistants and unit clerks.

Our nursing staff is well-educated and specially-trained in treatment measures such as wound care; comfort care; and pressure ulcer prevention, to name a few. 

While all staff members provide emotional support when needed, skilled counseling by social workers is available to residents and their families.
