The Evergreen Health Center is a 108-bed licensed
Medicare-certified skilled nursing facility
consisting of three areas - The Post Surgical and Rehab Center
at Manor View, Creekview North
and Creekview
South.
The Post Surgical and Rehab Center at Manor View provides care for 28 residents whose needs are focused primarily
on short-term rehabilitative therapies.

Creekview North provides care for 36 residents and Creekview South provides
care for 44 residents. Both offer a nationally recognized innovative approach
to long-term skilled nursing care. Each area contains four individual households that
provide an appealing homelike neighborhood setting.
The entire Health Center is Medicare-certified, is designed for residents who
need ongoing skilled nursing care, and is staffed by nurses and
certified nursing assistants
24 hours per day. Day and evening resident to nursing
assistant
staff ratio is 7:1. Overnight ratio is approximately 16:1. In
addition, other staff members may include Social Services
Specialists, Certified Occupational Therapy Assistants, and unit
clerks.
While all staff members provide emotional support when
needed, skilled counseling by social workers is available to residents and
their families. A special support group is also available to families
of residents with Alzheimer’s disease. Nurses have special advanced
end-of-life training to better equip them to care for
hospice residents.
Seventy Health Center rooms are private and 19 are semi-private. All rooms
have a half-bath. Each area includes a dining room and comfortable lounge
with a fireplace, an aquarium and aviary. The Post Surgical and

a library with books and audio cassettes on loan from the Oshkosh
Public Library. Creekview North
and Creekview South have play areas for young visitors and fenced-in outdoor courtyards and
both are close to Creekview Center and
Creekview Café.
Health
Center residents are encouraged to participate in activities
appropriate to their abilities and are welcome to attend facility-wide events. For
residents unable to participate in groups, staff members provide individualized
interaction. In addition, Health Center residents can participate on the
Residents Council or other committees, volunteer their talents, and interact
with children of many ages through planned activities.

- The Elizabeth J. Rasmussen Fitness Center contains
specially designed strength and
cardiovascular equipment. Personalized fitness programs and group fitness
classes are offered.
- Open swim and group swim classes are held in the two
Aquatic Center therapeutic
pools. The John J. and Ethel D. Keller Pool offers opportunities for lap swimming and aerobic conditioning. The
Prescott Family Pool warmer water allows for more gentle exercise.
Both pools are cleansed with the newest
cutting-edge ultraviolet technology. An overhead lift is available for those
unable to use the ramp or steps to enter the pools. Unique individual unisex changing rooms
offer the privacy of a complete bath with a no-step shower and adequate space for assistance.

The Aerobic Studio affords opportunities for strength and balance training and
specialized arthritis classes. Its special shock-absorbent flooring offers support
for joints during high and low impact aerobics.
- Evergreen Therapy Services include physical, occupational, and
speech therapies along with other restorative programs.
